- Summary: INABILITY TO STOP MOVING VEHICLE CAUSING AN ACCIDENT. THIS VEHICLE IS A 1995 CHEVY LUMINA MINIVAN. DRIVING 55 (POSTED SPEED LIMIT) DOWN A RURAL HIGHWAY AND APPROACHING STOP SIGN, THE VEHICLE WOULD NOT SLOW DOWN FAST ENOUGH TO MAKE THE STOP, FORCING EVASIVE ACTION BY DRIVER AND AN ONCOMING, CROSS TRAFFIC VEHICLE. THE BRAKES WERE FOUND TO HAVE 75% AVAILABLE PAD AND WERE IN EXCELLENT WORKING ORDER. THE ANTILOCK BRAKE SYSTEM WAS FOUND TO BE IN WORKING ORDER, RESPONDING APPROPRIATELY UNDER CIRCUMSTANCE. THE TRANSMISSION IN THIS VAN WAS COMPLETELY REBUILT BY A REPUTABLE TRANSMISSION REBUILDER WHO IS REGISTERED WITH THE AUTOMATIC TRANSMISSION REBUILDERS ASSOCIATION IN OCTOBER 2002. A THROTTLE PROBLEM OCCURRED SHORTLY AFTER THE TRANSMISSION WAS REBUILT, AND THE PROBLEM WAS FIXED UNDER WARRANTY. THAT PARTICULAR TIME THERE WAS SIMPLY A SHUDDER WHEN GOING FROM 4TH TO 5TH GEAR, AND THERE WAS NO THREAT TO SAFETY KNOWN. BY THE TIME SHE WAS ABLE TO GET TO A SAFE PLACE TO PARK THE NOISE HAD STOPPED. SHE WALKED AROUND THE VEHICLE LOOKING FOR ANYTHING OUT OF ORDER AND SAW NOTHING. SHE GOT BACK IN THE VAN AND BEGAN DRIVING, THE NOISE STILL HAD NOT RETURNED. AS SHE WAS ABOUT TO GET BACK ON THE PARKWAY THE RIGHT FRONT WHEEL OF THE VEHICLE SUDDENLY FLEW OFF OF THE VEHICLE. SHE WATCHED THE TIRE GO STRAIGHT UP INTO THE AIR, "VERY HIGH" AND CAME DOWN BOUNCING ON THE STREET IN FRONT OF THE VAN AND THEN GOING TO THE SIDE OF A ROAD AND ROLLING UP AN EMBANKMENT BEFORE IT STOPPED. WHEN THE TIRE FLEW OFF THE VAN SLAMMED DOWN INTO THE STREET WITH GREAT FORCE. THE IMPACT OF THE WHEEL FLYING OFF AS WELL AS THE VAN SLAMMING INTO THE STREET CAUSED THE ENTIRE SIDE PANEL OF THE RIGHT FRONT SIDE OF THE VAN FROM THE DOOR TO THE HEADLIGHT TO SHATTER ALL OVER THE STREET. THE VAN WAS TOWED TO THE DEALER WHICH IS WAS PURCHASED FROM 30 DAYS EARLIER AND WE WERE TOLD BY THE SERVICE DEPARTMENT THAT THERE WAS A "DEFECT IN THE LUG NUTS" WHICH CAUSED THE WHEEL TO FLY OFF. EACH LUG NUT HAD MADE A CLEAN BREAK CAUSING THE WHEEL TO FLY OFF. PLEASE LET US KNOW IF YOU HAVE RECEIVED ANY OTHER COMPLAINTS FOR THIS TYPE OF INCIDENT.*AK

- Summary: VEHICLE DESCRIPTION: PASSENGER VEHICLES MANUFACTURED FROM AUGUST 1991 THROUGH JULY 1995. THE TRANSMISSION OIL COOLER LINE CAN SEPARATE WHEN THE VEHICLE IS OPERATED IN LOW TEMPERATURES IN COMBINATION WITH HIGH TRANSMISSION LOADS.
- Consequence: VEHICLE FIRES CAN OCCUR DUE TO THE SPILLAGE OF TRANSMISSION FLUID.
- Remedy: DEALERS WILL REPLACE THE TRANSMISSION OIL COOLER LINES.

- Summary: THESE VEHICLES WERE ASSEMBLED WITH THROTTLE CABLE SUPPORT BRACKETS THAT COULD CONTACT THE THROTTLE LEVER SYSTEM AND INHIBIT THE THROTTLE RETURN. THIS DOES NOT MEET THE REQUIREMENTS OF FMVSS NO. 124, "ACCELERATOR CONTROL SYSTEMS."
- Consequence: CONSEQUENCE OF NON-COMPLIANCE: THE THROTTLE MAY FAITL TO RETURN QUICKLY CAUSING THE ENGINE SPEED TO NOT DECREASE TO A LEVEL ANTICIPATED BY THE DRIVER.
- Remedy: DEALERS WILL REPLACE THE THROTTLE CABLE SUPPORT BRACKET.

- Summary: IMPROPERLY MANUFACTURED BRAKE PEDAL ARM CAUSES THE BRAKE PEDAL ARM TO FRACTURE DURING BRAKING.
- Consequence: THIS CONDITION CAN RESULT IN A LOSS OF BRAKE OPERATION INCREASING BRAKING DISTANCE AND THE POTENTIAL FOR A VEHICLE ACCIDENT.
- Remedy: DEALERS WILL REPLACE ALL SUSPECT BRAKE PEDAL ASSEMBLIES.
